MEMBERSHIP BENEFITS & ELIGIBILITY

Membership benefits are available to active members in good standing during the term of their membership only. If your membership ends, terminates, or is otherwise not in good standing, you will be prohibited from using any such benefits.

ONE-TIME SETUP FEE

A one-time setup fee of $50 is required upon enrollment into The Smooth Society membership program.

MEMBERSHIP CREDITS & PAYMENT

A monthly payment is charged via EFT (electronic funds transfer) beginning 30 days from the day you sign up and continues monthly on your anniversary date until termination. Each month an EFT is charged to your account you receive a credit under your account. This credit is available for you to redeem for a treatment or for retail based upon scheduling and availability.

MONTH-TO-MONTH COMMITMENT

The Month-to-Month program gives you the flexibility to continue for as many months as you wish without any long-term commitments. Cancellation requires 30 days prior written notice any time after your first monthly payment. There is a 90-day minimum membership commitment. If you desire to terminate before your fourth monthly payment is drawn, you will be required to pay the fourth month, and then your membership will terminate 30 days thereafter.

To terminate your membership, you must give written notice to Hello Smooth Med Spa and receive written confirmation of such termination in order for such notice to take effect. Hello Smooth Med Spa reserves the right to terminate any membership for any reason at any time without notice or refund.

ADDITIONAL PURCHASES

Members may purchase an unlimited number of additional services and products for their personal use at the discounted rates given within their membership level. Discounted services may not be scheduled or transferred to any one else for any reason at any time. 

ROLLOVER PRIVILEGES

Members may rollover any unused credits to the following month. Upon termination of your month to month membership, accumulated member credits expire and are non-refundable 90 days after you cancel your membership. Additionally, these credits may not be refunded, withdrawn or transferred. 

RETAIL DISCOUNTS

A 10% retail discount is given to Silver and Gold members and a 15% retail discount is given to all Diamond members on skincare and retail products. Members must be current and discounts only apply during the Term of your membership.

It is the Member’s responsibility to notify Hello Smooth Med Spa about any changes in address, contact information, and/or credit card information. If your payment method declines, your membership benefits shall cease after 2 attempts to collect payment have failed. 

Guests must follow spa directives, rules, regulations and policies, as amended from time to time. Hello Smooth Med Spa reserves the right to refuse or discontinue services, loyalty rewards or membership benefits for any reason.

Hello Smooth Med Spa reserves the right to modify/change spa rules, policies, regulations, services, membership benefits and pricing with reasonable notice.

GENERAL POLICIES

  • Memberships are non-transferable and non-refundable.
  • It is the Member’s responsibility to notify Hello Smooth Med Spa about any changes in address, contact information, bank wire information, and/or credit card information.
  • If your payment method declines, your membership benefits shall cease after 2 failed attempts to collect payment.
  • Guests must follow spa directives, rules, regulations, and policies, as amended from time to time.
  • Hello Smooth Med Spa reserves the right to refuse or discontinue services or membership benefits for any reason.
  • Hello Smooth Med Spa reserves the right to modify/change spa rules, policies, regulations, services, membership benefits, and pricing with reasonable notice.

BOOKING POLICIES

  • Cancellation Policy: We have a 24-hour policy for late cancellations. Cancellation notifications given less than 24 hours prior will result in a non-refundable fee of $50.
  • Late Policy: Our late policy allows a 15-minute grace period. If you are more than 15 minutes late for your appointment, you will need to reschedule and will be subject to a non-refundable fee of $50.
  • No Show Policy: Failure to show for any appointment will result in a non-refundable fee of $100.
  • Communication Policy: Please only call or email us to inquire about scheduling, rescheduling, or canceling an appointment. Sending a message through social media is not considered an appropriate form of scheduling, rescheduling, or canceling.
  • All services have an expiration date of 365 days from date of purchase.

PRE-PROCEDURE POLICIES

  • Please arrive makeup-free for the following services: Facial Laser Hair Removal, Facial Filler Injections, Facial Skin Resurfacing, Botox/Dysport, Facials.
  • Always review your pre and post care for more detailed instructions on each specific service. If you are unsure where to find this, contact our office at 904-342-2383.

AGE REQUIREMENTS

  • Dermal Filler: Must be 21 years or older to receive filler treatments.
  • Botox or Dysport: Must be 18 years or older to receive Botox or Dysport treatments.

Membership Credit Usage:

  • Monthly membership payments are credited dollar-for-dollar to the member’s account.
  • Credits may be used toward eligible services and retail products.
  • Credits cannot be redeemed for cash.
  • Credits are non-transferable.
  • Credits remain available while membership is active and expire 90 days after cancellation.
